A normal shoulder has the ball of the joint on the humerus and the socket on the scapula, in this case, the artificial joint is reversed.

He had to do this due to the rotator cuff being so thin and damaged from 7 other surgeries, somehow, reversing the shoulder anatomically somehow allows for more strength.
